**Important Note!

As you order this book, you'll be asked four things that will go into the 'personalization' of the book:

• Your first name (or the most commonly used variation--for example, if people call you "Dan" you wouldn't want to put "Daniel" as first name)

• The name your mother generally calls you.  For most people, that would be the same as the first name, but occasionally mothers have their own nicknames for their kids, so we've included this in order to 'deepen' the personalization.
  
  
• A nickname that might have been used in your childhood (say, before the age of 14).  For most people, this would be the same as the first name, but we've included it for people who generally go by a different name when dealing with friends.
